Review button shows up sometimes but not for every student

Our client who uses LearnPress has alerted me that when her students take the quizzes, the review button no longer shows up. The quizzes are set to:

Allow students to review this quiz after they finish the quiz.
Show correct answer

AND

Allow students view correct answer question in review this quiz.`

But when both of these are checked it still does not generate the review button. I did find that if you put the answer in the “Explanation” field for each question then the review button appears- but it doesn’t show what was entered in the explanation box which is very odd.

The students use to be able to review- I believe this was before the last big update (cannot recall if this was for 4.1.2- it was the one that made me refresh the database).
