Form conditional redirect to post – Is it posible?

Hi

I would like to know if I can achieve this with your plugin.

I have one video per post. The post title is a code.

I will present a form to the visitor.

The visitor inserts the code in the form and submits it.

The visitor gets redirected to the post’s page (that has that code as the title).
