How to integrate these structured data (telephone, email, CreativeWork)

Hi everyone, I’m designing a custom WordPress theme, it’s a portfolio, and I’d like to ask you for help on how to integrate these particular structured data into Yoast SEO. I enter specifically.

In the case of itemtype=”http://schema.org/Organization” I would like to integrate these two properties: itemprop=”telephone” and itemprop=”email”.

Also inside the homepage and the single project page I would like to integrate itemtype=”https://schema.org/CreativeWork” and the related properties.
