Mysterious, deafult checkbox which appears at the top of each form

Dear Support team
I would ask for help with the HappyForm – with the deafult checkbox that appears at the top of each form [just from the beging]You can look at it at the page:http://pracownia-aw.pl/wp-arkhitekton/wp-admin/post.php?post=24427&action=elementor# { i put 2 Happy forms- to show you the thing: it is easily visible on TEST form, but it appears in each one, as it shows the second example- with the Sample Form.

I am using the free plugin, and I have checked it with differen version of plugin- 1.9.9 and with the newest one 1.9.10.
But this misterious checbox remains, I have checked that plugin on other website and it works perfectly.

Could you help me with instructions how to remove this “mysterious checkbox”
