retrieve image from form and save to a folder in database

Hi,

I would like to know the procedure in using this plugin to save an image that is uploaded in a form and save this image to a specific folder in the database. I found the code here https://cfdbplugin.com/?page_id=794 and adjusted the $formName, $fieldName and $uploaddir variables. I then saved this code in Tools -> shortcodes, actions and filters

What do I do next to make this work?
