unable to add “name’ field to the form

hello, thanks for your awesome plugin. i want to add NAME field to my signup form, so that user can input their name and email during sign up. i tried to do this on the plugin add form page which directed me to mailchimp audience dashboard to add new field to the form but there is no NAME field available, please how can i do this? thanks for your reply in advance.
