Form Submission Failing/Refreshing Page (Urgent issue)

Hi,

I’ve added a contact form 7 form into a php template using <?php echo do_shortcode( ‘[contact-form-7 id=”887″ title=”become-a-client”]’ ); ?> and it displays perfectly fine.

I’m able to add data into the fields and compete it as expected. When I click the send button however the page simply refreshes and put a bunch of data into the url as seen below.

Help urgently needed as this is work we’ve been trying to complete for a client of ours.
