Show Form in Excerpt/show shortcode in Excerpt

Hello,

I’d like to show a basic form in my excerpt that users can fill in. If it’s possible to display a shortcode either as a excerpt or in my excerpt that’s also ok.
Is that possible? Someone here to help me out?
